Sworn Statement Form.
The Administrative Contracting Advisory Board has approved the standardised form for formalising the written sworn statement to be completed by the bidder and submitted to the contracting body declaring there have been no changes in circumstances with regard to legal status and any other circumstances that must be included in the Official Register of Bidders and Classified Companies.
Once completed, the form can be printed and signed by hand, or saved in xml format (Menu ""File"", option ""Save as"", select""Save as: XML document""), and then signed electronically and sent to the contracting authority, in accordance with its instructions on the places where, and means by which sworn statements can be submitted.
